Types of Leave

 

  • Pregnancy/Birth of Child/Adoption
  • Care for a Child with Illness
  • Care for a Parent with Illness
  • Care for a Spouse with Illness
  • Own Illness/Injury
  • Personal *not eligible for FMLA*

Steps to Take When Requesting a Leave of Absence

  • Notify your Operations Manager at least 30 days in advance of your leave. If 30 days’ notice isn’t possible, notify them as soon as you become aware of the need for leave.
  • Complete the FMLA paperwork provided by email from ADP (if applicable).
  • File a short- or long-term disability claim through MetLife if you are eligible (see details below).
  • Submit a return-to-work release to your Operations Manager (required for all medical and maternity leaves).
  • Coordinate with People Operations and your Office Manager regarding any work restrictions or accommodations.
  • Return to work, and we’ll support your smooth transition back!

FMLA (Family Medical Leave Act)

Employee Eligibility

1. Worked for the company for at least 12 months

  • The 12 months do not have to be consecutive

2. Worked at least 1,250 hours in the 12 months prior to the leave

  • Includes only actual hours worked (not PTO, holidays, etc.)

FMLA Leave

1. Up to 12 weeks of Job-Protected Leave

  • If eligible you can take up to 12 weeks of unpaid leave in a 12-month period for qualifying reasons (see above). PTO, Short-term Disability and/or Long-term Disability for income replacement during your leave. *see claim submission and approval process for STD & LTD below*
  • This leave is job-protected, meaning that you will be reinstated to the same or an equivalent job when you return.

2. Continued Group Health Insurance Coverage

  • During FMLA leave, any benefits you are enrolled in will continue under the same terms as if you were still working.
  • You will be required to pay any missed benefit deductions upon return from leave. If you do not return from leave, you are still expected to pay the missed deductions.

Documentation

  • You are required to provide supporting documentation for the leave (e.g., doctor’s note). And you and your provider will need to complete a Health Certification to ADP to be reviewed for approval.
  • For all medical related leaves, a return-to-work release will be required before you can resume duties.

Company-Granted Personal Leave (Non-FMLA Leave)

If you are not eligible for FMLA, the company may still approve a personal leave of absence, depending on business needs. Here’s how it works:

 Practice-Discretionary Approval

  • Leave may be granted if the practice can support the absence without disrupting operations.
  • Approval is not guaranteed and is based on coverage, staffing, and timing.

Unpaid Leave

  • This leave is unpaid unless you use PTO, Short-term Disability and/or Long-term Disability for income replacement during your leave. *see claim submission and approval process for STD & LTD below*

No Job Protection under FMLA

  • Since it’s not FMLA-protected, job reinstatement is not guaranteed, but the company will make every effort to return you to the same or similar role if available.

Health Benefits

  • Your benefits will end on the last day of the month in which the leave begins.
  • You will receive a COBRA notice and may elect to continue their benefits at your own expense.
  • You will be able to re-enroll in benefits upon return to active employment.

Documentation

  • You are still required to provide supporting documentation for the leave (e.g., doctor’s note).
  • For all medical related leaves, a return-to-work release will be required before you can resume duties.

How to File a Short-Term or Long-Term Disability (STD/LTD) Claim

All disability claims are administered through MetLife. For the purpose of filing your claim, our company is listed under ADP TotalSource

Eligibility Reminder

  • Company-Paid Short-Term Disability (STD):
    Provided to all full-time employees at no cost.
  • Company-Paid Long-Term Disability (LTD):
    Provided to full-time employees enrolled in a company medical plan.
  • Voluntary STD/LTD Coverage:
    Available to any employee who elected coverage during open enrollment.

Ways to File a Claim

You can file your claim using either of the following methods:

1. Online

  • Visit: www.metlife.com/mybenefits
  • Create or log in to your account
  • Select “Start a Disability Claim”
  • When prompted, enter our company name as:
    ADP TotalSource

2. By Phone

  • Call MetLife at 1-800-300-4296
  • Provide your personal information and workplace details
  • Be sure to state the company name as:
    ADP TotalSource

What You’ll Need to File

  • Your personal information (name, address, phone, date of birth)
  • Your employee information (hire date, job title, work schedule)
  • Details about your condition and anticipated time off
  • Your doctor’s information (MetLife may contact them directly)
  • Any documentation related to your medical condition

What Happens Next

  • MetLife will reach out if they need additional documentation from you or your provider.
  • MetLife will reach out the Lone Peak People Operations Team for details about your employment and leave.
  • Claims are typically processed within 7–10 business days, though timelines may vary.
  • You’ll be notified directly by MetLife with your claim status and payment details (if approved).

State Program Resources

If you are an employee in the state of Colorado or Washington, you may be eligible for paid leave through the state. Other states may have unpaid FMLA programs, please check your states website for details.

Colorado FAMLI (Family and Medical Leave Insurance)

1. Eligibility:

  • Work in Colorado and earned at least $2,500 in the past year.

2. Create an Account & Apply:

  • Use the My FAMLI+ portal at the Colorado Department of Labor and Employment myfamliplus.state.co.us.
  • You can apply before or once leave begins.

Washington PFML (Paid Family & Medical Leave)

1. Eligibility:

  • Employees (full-time, part-time, seasonal) who worked at least 820 hours (~16 hrs/week) in the prior year qualify.
